Online forms

The following forms can be completed directly online, without needing to download anything or post anything.

  • If you are moving house, you can notify us of your new address using the online form.

  • The tax statement for EGK health insurance can be requested online. It will be sent via myEGK (app or portal), by email (HIN) or by post.

  • In the event of a claim, treatment costs should be reimbursed as quickly as possible. To ensure efficient processing, the accident must be reported to EGK or the relevant authority via the accident report form.

    Who to report the accident to:

    • If you work for the same employer for more than 8 hours a week, their accident insurance is responsible for both non-occupational and occupational accidents.
    • If you work for the same employer for fewer than 8 hours a week, you are insured against occupational accidents whilst at work, including on your journey to and from work. Non-occupational accidents must be reported to your health insurance provider.
    • Children, people not in employment and pensioners are covered against accidents through their health insurance.
    • If you are receiving unemployment benefit, accident cover is provided by Suva. Enquiries regarding this should be directed to the unemployment benefit office.
  • If you wish another person to manage your insurance matters, you can grant them power of attorney.

    How do I notify EGK of a change of address?

    You can notify us of a new address directly via the ‘Change of Address’ online form, without needing to use a paper form or ring the agency. Registered users can also update their address directly in the myEGK app.

    Who should an accident be reported to – the health insurance provider or the employer?

    This depends on the level of employment. If you work more than 8 hours a week for the same employer, their accident insurance covers both work-related and non-work-related accidents. If you work fewer than 8 hours a week, cover through your employer applies only to work-related accidents, including those occurring whilst travelling to and from work. Non-work-related accidents must be reported to your health insurance provider. Children, people not in employment and pensioners may be covered against accidents through their health insurance. People receiving unemployment benefits are insured through Suva.
